It/they are making a flippin mess I've never seen mine eaten in this way, it looks like the work of some sort of caterpillar to me, have you had a good look under the leaves

Have you seen any small beetles on them. Some species of shrub or tree such as Willow or Vibernum has an associated beetle that eats them.
Catterpillers, snails and vine weevil beetles could all be guilty although ive never know Hydrangeas to be affected by slugs and snails
